Applicants are required to provide detailed information regarding their facilities, operational procedures, and the need for qualified halal supervisors.

The completed form is submitted as a PDF document and enables ICH Halal to assess operational compliance, assign halal supervisors, and issue the required voucher certification where applicable.

After completing the online application form, please print the completed form and sign it.
Attach one recent passport-size photograph (white background, passport standard).
